    Let us know how she does long term. I would expect this to come back quicker then an apthous leasion.

    I was asked a question today and really did not know the answer.
    How does the laser actually effect pain relief with oral ulcers, ie what is the mechanism of action. Are we destroying nerve endings, creating a coagulum type bandaid? Anyone with any laser , thanks?

    Yeah, pretty much. In addition, heat photons dentaure proteins, certain wavelengths biomodulate unmylenated c-fibers by changing the cell membrane’s permeability to Ca and K, perhaps. N-IR lasers reduce the inflammatory response, by interferring with the Arachodonic cascade, hence reduceing the bradykinines and cytokines, which themselves are heat labile……..just some of my thoughts from my readings over the years.

    1 week follow up on patient with paraneoplastic pemphigoid.
    Patient gleefully reported that this is the first time in ten years she is pretty much pain free in mouth. She had me do a few more ulcers that bothered her but was confident that it would feel even better tomorrow.
    As she is in the area once a month I expect to see heragain.

    Good for her. It’s great when spmething relatively simple is done to bring about a drastic improvement for the patient.
