    Hey, Graeme! You out there?

    I just bought Graeme’s latest CD on Auto-Cure GI’s and how to place them. Fantastic information for a cheap price! I know that Graeme always suggests using the 10% poly-acrylic acid to remove the smear layer.

    I say – What ever creates the best bond between the tooth and the GI, I’ll do it.

    Just a pitch for Graeme Milicich: I have all of his Microdentistry CD’s – They’re awesome! They really help staff and patients understand the benefits of this approach to dentistry.

    I’m gonna toss my hat in the ring too.

    How conservative is a restoration if it has to be re-done. I understand that the only permanent tx we do as dentists is extraction. As Mossman pointed out, how conservative were his initial preps when they had to redone due to marginal ridge fracture? Inevitably, these preps get larger each time their re-done. I think this is a great learning discussion.

    Doug, I agree. This is a great discussion. I think it brings about another important point: The discussion about CAMBRA. Since we started talking frankly to patients about the bacterial infections they have that have been untreated throughout their lives, it really helps patients understand all of the factors influencing the caries process. I also love giving patients a “prescription” of how to treat their infection. We are starting to see great results in reducing the bacterial colonies of Mutans Streptococci and Lactobacilli as we continually monitor their levels with the CRT bacteria system.

    Hopefully in time more practices will emphasis this medical model to their patients. No sense putting a new roof on a burning house!

    CAMBRA….

    I’m…(gulp!)…not familiar with CAMBRA….but I’ve gathered it is a philosophy of treated dental caries as an infection. Utilizes bacterial assays, medicaments, etc. Enlighten me – any good online sources? My interested is peaked.

    I’ve been out 7 years and have learned more on DT and here in 7 months than all 7 years together!

    Would you say you routinely remove less overall tooth structure when doing a tunnel prep vs. the most conservative slot prep/removing the marginal ridge? Recently, I’ve been amazed at how I thought I was going to have a very small conservative tunnel prep on a very similar lesion, only to find out that the overall volume of tooth structure removed is about the same had I removed the marginal ridge.
    This brings up another question . . . . why not just allow that lesion to grow a little more (I’m I really saying this?) and then restore it once the marginal ridge absolutely cannot be saved . . . . then place an indirect restoration there and potentially get 20-30 years out of it? That way you avoid getting into the tooth twice, and you’ve given the patient a great restoration (be it Cerec, gold, or others) that we have more confidence in? (I’m playing devils advocate a little here, if you can’t tell . . . .)
    Any thoughts out there?
