Haven’t seen this patient in seven years. Forty something Male. Presents with broken Maxillary canine and alot of unesthetic dentalwork and other problems.
    Tooth #11 has had RCT.

My treatment will be laser gingivoplasty with a pulsed Nd:YAG laser, post and core build-up, immediate final impression for a CAPTEK crown and construction and cementation of a temporary crown.
PerioLase was used 3.0W 20Hz 100usec to remove overgrown gingiva, trough for impression and establish proper gingival contours. Total exposure less than a minute. Note condition of gingiva after lasing. No charring, no bleeding.

Tissue was further retracted using a Roeko cotton compression cap, no retraction cord or hemostatic solutions. Impression with Impergum Soft. A polycarb crown was made.
Patient told to take Motrin a couple of days, salt water rinses if needed.
    2 weeks later-

    Healing with temporary in place.

Condition of tissue at 14 days.

    Immediately after crown insert.
Note position of crown margins vs. gingival margins.
It’s nice to be confident that there will not be recession after gingivectomy or gingivoplasty.
Will follow-up with another post-op, if possible.

does anyone else question the long term longevity here?

I have no opinion on the use of the laser here..it did fine, just long term prognosis with that occlusion on that tooth with a root canal and little clinical crown.

    Post-op photo at 3 months.
Note health of tissue and no recession despite taking impression at time of gingivoplasty with PerioLase.

    Bob,
I’m inserting a clear acrylic anti-bruxxing appliance on Mandibular arch next week. To try to prevent future fractures.
    Looking good.
Too bad the case wasn’t done with the WaterLase, then it would have been a case with more discussion.
    Thanks.
My best wishes for a great Christmas!
