    Great Idea Ron. Talked with her tonight. Better by far but now her discomfort is futher back. I’ll give it a try.
    What/Who are you reading on Biostim?
    I hate quoting articles! I have a tough time remembering names etc. I remember the important stuff and move on. Looks like this will be something in my past. Starting to get some local flak about using the laser. Wondering if we could start a thread that just list articles that were of relevance. Seems like it could be a good idea. If there is already a spot on the board like this, forgive me a point me in the right direction!

    Hi Ken,

    I believe the original question was regarding tissue response after laser treatment and if it differs from the conventional methods.

    I do a lot of preprosthetic treatment with conventional resective surgery and occasional electrosurge. No experience with the diodes.

    By the way, very nice improvement on the case you posted!! Too bad she wouldn’t go for the ridge augmentation.

    Hello All: I read this diode discussion with a lot of interest and now much frustration and confusion. So here’s my two cents:
    When I study some science (thanks, Bob Gregg, my friend!) I find that the water absorbance of the short wavelength lasers (pick a number between about 500 and 1000nm) is very little, especially when you compare the 10 to the 4th power absorbance of the Erbium family. To me that means that the water absorbance is next to nothing. The area around 1000nm is about 2-3 times that of the area around 800nm, but that’s still 2-3 times almost nothing. The advantage to using those shorter wavelength lasers is that you CAN work in a somewhat wet field if you need to because the laser light transmitts through the water (which of course is why it’s not absorbed.) The purpose of “activating” or “initiating” the tip of a diode device is to start some thermal activity at the point of contact with the tissue, so that you can begin to ablate. The tip is supposed to be very lightly “carbonized, ” not “charred.” The diodes have tiny peak powers compared to the free running pulsed machines, like the FR Nd:YAG, so that’s why you would initiate the tip. If you wish the diode energy to penetrate into the tissue to achieve, for example, hemostasis or aphthous ulcer decontamination, you do NOT initiate the tip. The nice thing about your ability to use some wetness in your diode surgery is that you can cool the target tissue, since you’re using an essentially continuous wave output. Those of us who grew up on FR Nd:YAG’s have to be very careful in using the continuous wave devices because of that tissue cooling problem. Anyway, from all my reading, practicing, and writing, I conclude that, for all diodes, 1) water absorbance is meaningless, 2) you initiate in some cases and not in others, 3) be careful of advertisements if there’s no science.
    Your biological width observations are good ones, by the way. My only addition to the discussion is to once again emphasize that the continuous wave emission of laser energy can get you into some trouble when the tissue cannot dissipate the heat fast enough. Thanks for reading this far……..DON

    called biolase last week told them that i got a badbatch of tips so they graciously sent me out some new ones at no charge…used on the the new tips today and BOOM… first time sparks came out and then changed tip used another got the tooth done only for the fiber to conk out right at the end of the treatment…. I just dont understand why they cant stabilize the consistency of the tips……Now I got to go plead my case to the powers at be

    Here is the picture I promised of the Karl Shumacher #045 spoon excavator.
    They run about &#3620 apiece. I got 6+1 at the DC Meeting in March.
    I use for final caries removal w/ the Erbium.

    What is the definition for a bad tip?

    What kind of problems can be the cause?

    What is your take on this?

    What is the explanation that has been given by Biolase?

    I have just had my new Waterlase for 1 week.This was just delivered to me by the technician who did not know any clinical info at all.
    I was told by the agent in the UK that my clinical training can be to watch a dentist who has been using his waterlase only for a few weeks.This dentist told me he will call me when he has a few cases and I can watch him,but once you have a waterlase in your surgery you do not want to wait a few weeks.
    Hence I had to train myself to use the laser.I got a clinical guide by Chris Walinski which was very helpful,and a DVD of clinical applications which were recomended in this forum and were very good.
    After 4 days I prepared an occlusal cavity on a patient when I smelt burning.No water came out.No spak either.I called Biolase rep in UK who said they will send a technician 3 days later.He said the G6 tip was dark(but not broken) and the fibre is damaged together with the handpiece mirror;He replaced this and the laser is ok now.
    After all that boring story(I doubt anyone has read this far?) my question is do you experienced users think I am responsible for paying for the damage?
    I think that the POOR help I got (or did not get) from the Biolase rep in UK i.e lack of minimal training places them to be responsible for the cost.

    Mark may know better but they have always had problems with bad batches of tips.. Usually u get a spark and u know here comes trouble…I hope they will be good in correcting my problem

    mick, i wold be surprised if they actually charged you, but i dont think u should pay..if u have any laser questions feel free to call me here in NY at 516 239 6204..I’ve been using the laser for almost 3 years and have been thru it all…Regards Ron
